Low Back Pain Treatment in Fort Myers & Cape Coral Without Surgery or More Pills

60-minute one-on-one sessions to get you sitting, working, and training without fear.

No doctor’s script needed60-minute 1-on-1 visitsDay-one diagnostic testing

If your low back pain keeps coming back every few months, this is for you.

You’ve probably tried “resting it,” Googling stretches, maybe a few rounds of meds or injections. It helps a bit, then the pain returns the moment you sit too long, stand too long, or try to work out again.

At Apex, we work with people in Fort Myers and Cape Coral who are tired of chasing temporary relief and want a clear plan to get back to normal life without living at the doctor’s office.

Who this is for

What kind of low back pain do you have?

Most people we see fall into one of these buckets.

New Flare-Up

Less than 6 weeks
  • Started after lifting, bending, or a random “tweak”
  • Feels stiff, sore, sometimes sharp with certain movements

On-and-Off Pain

More than 3 months
  • Comes and goes based on work, stress, or workouts
  • Mornings and end of the day feel worst

Back Pain With Leg Symptoms

Sometimes called “sciatica”
  • Pain, tingling, or numbness going into the butt, thigh, or down to the foot

Red flags: see a doctor or ER first

  • Recent major fall or accident
  • Sudden loss of bladder/bowel control
  • Numbness in the “saddle” area
  • Unexplained weight loss plus night pain

If that’s you, do not wait. Get urgent medical care. For everyone else, Apex is usually the front-line option.

Apex physical therapist evaluating a patient
Your first visit

What happens in a low back pain evaluation at Apex?

A typical first visit is about 60 minutes and includes:

  1. Conversation

    5–10 minutes

When it started, what makes it better/worse, and what you need your back to do (lift kids, work, train, etc.)

  2. Movement and strength testing

    5–10 minutes

How far you can bend, twist, and extend. Which positions increase or decrease your pain. Core, hip, and leg strength.

  3. Targeted hands-on testing

    5–10 minutes

Specific joints and muscles around your spine and hips.

  4. Clear diagnosis and game plan

    5–10 minutes

What’s likely driving your pain, which activities to change right now, and how many sessions we recommend and why.

  5. Diagnostic testing

    5–10 minutes

MSK Ultrasound or EMG nerve test.

  6. Treatment

    Based on the findings above.

You leave knowing:

  • What’s going on
  • What you should do between sessions
  • What we will do each visit

No guessing, no “let’s just try this for a while and see.”

Visits & plans

How many visits does it usually take?

Everyone is different, but here are the typical patterns we see.

Your situationTypical visitsTimeframePlan that usually fits
Mild new strain4–8 visits2–4 weeks6-Session Plan
Recurring or long-term back pain8–12 visits3–6 weeks12-Session Plan
Back pain with sciatica symptoms10–12+ visits6 weeks12- or 21-Session Plan

That’s why we usually recommend PLANS of 6, 12, or 21 sessions instead of random one-offs:

Enough sessions to actually promote tissue healing and restore strength and movement
A clear start and finish line
Lower cost per visit than buying one at a time

If we don’t think you need a PLAN, we tell you. Some people just need a few focused sessions and a strong home plan.

When is cash-based PT worth it?

Cash-based PT usually makes sense if:

  • You want elite treatment services, NOT what the insurance allows
  • You’re losing time, money, or sanity because you can’t sit, work, or train normally
  • You’ve already tried “quick fixes” and the pain keeps coming back

People often compare the cost to:

  • Missed work days or missed workouts
  • Repeated copays for short, rushed visits
  • More imaging or injections that don’t change how you move

If you’re not sure it’s worth it, that’s exactly what the first visit is for: to see if your back problem is something we can actually change.

“I came to Apex for a partial torn rotator cuff and nerve pain down my leg from a lower back issue. Both issues have improved drastically and rapidly. I would have thought they were lying if they told me how fast they could help both of these. Nerve pain completely gone after about 3 or 4 sessions and huge improvement in shoulder mobility. The team at Apex is knowledgeable, professional and very kind. All of them have been top notch from the beginning. THANK YOU.”

Common questions we get about low back pain

Do I need an MRI first?

NO. Many people with back pain get better with movement-based treatment alone. If we see anything that suggests you need imaging, we’ll tell you.

Can PT really help if I’ve had pain for years?

YES. Chronic pain is more stubborn, but we see long-term cases improve all the time when we change how you move, load, and recover. The key is a clear plan and consistency.

Will I have homework?

YES. The work you do between sessions is what locks in your progress. We keep it realistic: usually 10–15 minutes per day.

How do you know if my sciatica is coming from bulging discs in my back?

We will perform an EMG/NCS nerve test that will show if the source of your back pain is from the disc level or from another source.

Do I need a script from a doctor to schedule a session?

NO. You can be treated in the state of Florida without a script or referral from a doctor.
