Shoulder Pain Treatment in Fort Myers & Cape Coral Without Surgery or More Cortisone Shots

60-minute one-on-one sessions to get you back to reaching, lifting, and sleeping without pain.

No doctor’s script needed60-minute 1-on-1 visitsDay-one diagnostic testing

If reaching into the cabinet, throwing, or sleeping on your side makes your shoulder complain, this is for you.

Most people we see at Apex have already tried rest, ice, bands, maybe a cortisone shot. It helps for a bit, then the pain comes right back when they try to lift, work, or play again.

At Apex, we help people in Fort Myers and Cape Coral who want a clear, step-by-step plan to fix shoulder pain and get back to normal life without guessing or bouncing between providers.

Who this is for

What kind of shoulder pain do you have?

Most shoulders we see fall into one of these buckets.

Pain With Reaching or Overhead Work

Rotator cuff / impingement-type
  • Pain lifting the arm, reaching overhead, or out to the side
  • Often worse with putting dishes away, dressing, or pressing in the gym

Pain With Loading or Sports

Tendinopathy / instability
  • Achy or sharp pain after throwing, serving, lifting, or pressing
  • Feels weak, “unstable,” or like it might slip or give way

Stiff, Limited Motion

Frozen shoulder / post-immobilization
  • Hard to reach behind your back or overhead
  • Night pain and trouble finding a comfortable position

Post-Injury or Post-Surgery

Rotator cuff repair, labrum, dislocation
  • After a fall, dislocation, or surgery
  • Weakness, stiffness, or fear of moving the arm normally

Red flags: see a doctor or ER first

  • Big trauma with obvious deformity
  • Sudden loss of ability to move the arm at all
  • Fever plus a hot, red, very painful joint
  • Sudden severe chest pain or shortness of breath (don’t blame the shoulder here)

If that’s you, do not wait. Get urgent medical care. For almost everyone else, PT is usually the front-line option.

Apex physical therapist evaluating a patient
Your first visit

What happens in a shoulder pain evaluation at Apex?

A typical first visit is about 60 minutes, one-on-one, and includes:

  1. Conversation

    5–10 minutes

How it started, what makes it better or worse, and what you need your shoulder to do (work, sport, lifting, kids, etc.)

  2. Movement and strength testing

    5–10 minutes

How far you can raise, rotate, and reach with the arm. Strength of the rotator cuff, shoulder blade muscles, and upper back. How your neck and mid-back move (they change shoulder load).

  3. Targeted hands-on testing

    5–10 minutes

Specific tendons, joints, and structures around the shoulder.

  4. MSK Ultrasound

    10–15 minutes

Like an MRI and X-ray all in one — to determine the integrity of the tendons, muscles, ligaments, and nerves.

  5. Clear diagnosis and game plan

    5–10 minutes

The likely source of your pain, what to stop, start, or modify right now, and how many sessions we recommend and why.

  6. Treatment

    Based on the findings above.

You leave knowing:

  • What’s going on
  • Exactly what you should do between visits
  • What we will do in the clinic each session

No guessing, no “let’s just see what happens.”

Visits & plans

How many visits does it usually take?

Every case is different, but here are the typical patterns we see.

Your situationTypical visitsTimeframePlan that usually fits
Mild overuse / impingement-type pain4–8 visits3–5 weeks6-Session Plan
Rotator cuff or long-standing shoulder pain8–12 visits6–8 weeks12-Session Plan
Post-op rotator cuff or labral repair12+ visitsSeveral months12- or 21-Session Plan

That’s why we usually recommend PLANS of 6, 12, or 21 sessions instead of random one-offs:

Enough volume to calm the tissue, rebuild strength, and change mechanics
A clear start and finish line
Better price per visit than buying sessions one at a time

If all you need is a few targeted visits plus a strong home program, we’ll tell you.

When is cash-based PT worth it?

Cash-based PT usually makes sense if:

  • You want full 60-minute sessions with a specialist actually working on and coaching you
  • Your shoulder is already forcing you to skip work, change workouts, or avoid things you enjoy
  • You’ve tried “wait and see,” simple rest, or generic exercises and it keeps coming back

People often compare the cost to:

  • Time off work or sport
  • Repeated copays for rushed visits where little gets done
  • More imaging or injections that don’t change how the shoulder moves

If you’re not sure it’s worth it, that’s exactly what the first visit is for: to decide together if a plan of care is likely to change your situation.

Kylie, Apex patient and Auburn University softball player

Kylie’s story

Softball Player for Auburn University

Kylie first came to us in high school with a shoulder injury that was keeping her off the softball field. We got her in the same day, ran a diagnostic ultrasound, and discovered a tear. With targeted treatment, she was back to playing competitively within 8 sessions.

The result: she’s thriving as a college athlete — and still comes to us on her time off to keep performing at her best.

“I came to Apex for a partial torn rotator cuff and nerve pain down my leg from a lower back issue. Both issues have improved drastically and rapidly. I would have thought they were lying if they told me how fast they could help both of these. Nerve pain completely gone after about 3 or 4 sessions and huge improvement in shoulder mobility. The team at Apex is knowledgeable, professional and very kind. All of them have been top notch from the beginning. THANK YOU.”

Common questions we get about shoulder pain

Do I need an MRI first?

Not always. Many shoulder problems can be evaluated and treated based on your history and physical exam — plus our MSK ultrasound on day one. If we see signs you need imaging or a surgical opinion, we’ll tell you.

Can PT help if they said I have a tear?

Often YES. Many partial-thickness tears and age-related changes respond very well to targeted strength and movement work, without surgery.

Will I have exercises to do at home?

YES. The work you do between visits locks in your progress. We keep it realistic so you’ll actually do it.

Do I need a script from a doctor to schedule a session?

NO. You can be treated in the state of Florida without a script or referral from a doctor.

“This is the best team of therapists in the business. I had a partial tear in my rotator cuff. My range of motion was extremely limited. I had a complete tear 20 years ago and the surgery was brutal…”

“I injured my shoulders doing 100 burpees in January 2026… After trying chiropractors, cupping, infrared saunas… a trainer suggested to go to Apex… I felt 10% better after the first session. The diagnostic alone is worth it. The amazingly kind, fun, and proficient staff tailor the treatment to each individual.”

“I’ve been going to Apex Physical Therapy for treatment on my left shoulder, and I’ve been very impressed with the care I’ve received. The team has done an excellent job with my rehabilitation, including targeted exercises, stretching, and dry needling, all of which have made a noticeable difference.”

“I’ve had five visits to Apex so far and already have a greater range of motion than I’ve gotten in 15 sessions at another physical therapy place. I’d rather pay for this level of care than use what my insurance pays for. The ultrasound diagnostic was extremely helpful in pinpointing what is wrong with my shoulder. Alana, Mike, Keelan are awesome therapists and I highly recommend Apex.”
